The application for admission and all documentation is forwarded to the Dental Assisting Admissions Committee for Review.
- Official High School Transcript (translation may be required) or Official GED (scores needed)
- Applicants must be at least 18 years old on December 31st of that starting year.
- Application Form with $20 Application/Testing fee
- Admission Exam (ACCUPLACERÒ)
- One Letter of Reference is required
- Interview with Program Director/designee
- Student Health Record and Current Immunizations
- Proof of Health Insurance
- A satisfactory C.O.R.I. (Criminal Offender Record Information)
- Proof of Residency (in district)
