The application for admission and all documentation is forwarded to the Dental Assisting Admissions Committee for Review.

• Official High School Transcript (translation may be required) or Official GED (scores needed)
• Applicants must be at least 18 years old on December 31st of that starting year.
• Application Form with $20 Application/Testing fee
• Admission Exam  (ACCUPLACERÒ)
• One Letter of Reference is required
• Interview with Program Director/designee
• Student Health Record and Current Immunizations
• Proof of Health Insurance
• A satisfactory C.O.R.I. (Criminal Offender Record Information)
• Proof of Residency (in district)

Tuition in-district: $5,000.00
Tuition out-of-district: $9,000.00
